Name of Post: Hospital Attendant Grade – 2 Group D Staff

Age Limit: Candidates’ age must be between 18 years and 35 years. (Age relaxations will be extended as per Govt. rules).

Educational Qualification: Candidates must be pass outs of SSLC or 10th Class.

Application Fee Details: Applicants need to pay application fee of Rs. 100/- for SC/ST/Cat 1 candidates and Rs. 300/- for all other candidates through Challan/Bank Draft at Corporation Bank Account, Health and Family Welfare Services, Payable at Bangalore or at any NEFT enabled Bank Name of the Bank: – Corporation Bank Branch: – FCS-Bangalore, IFSC Code- CORP0000633, Account No.R4942 ———– (Application No.).

How to Apply: Candidates need to apply online for desired post on or before 30-06-2012. After submission of Online Application candidates need to make payment as per the details given above.

Instructions for Online Application:
1. Before starting the online application process candidates must apply scan their photograph and signature.
2. Then to start the application process log on to KarHFW website www.web5.kar.nic.in/hospital_attendent.
3. Select the Submit Fresh Application link to open online application.
4. Fill all mandatory details carefully, upload photograph and signature and click on Submit button.
5. Take print of Challan or NEFT Challan using the system generated Registration ID and Password.
6. Make the payment as per details given above.
